A 68 years old male who presented upper right back skin lesions with mild itching. which rapidly evolve to multiple sites. physical examination findings showed bilateral groin area and bilateral axillary lymphadenopathy. Bone marrow aspirate by Wright’s stain showed blastoid cell increasing(see Figure), account for 18% plus or minus. The lymph nodes biopsy demonstrated blastic plasmacytic dendritic cell neoplasm. Immunohistochemical staining: LCA(+++), CD(few+), CD20(-), CD45RO(few+), Bcl-2(++), CD5(few+), CD43(+++), CD35(-), CD21(-), CD117(-), CyclinD1(-), CD68(+), MPO(-), CD99(-), CD138(-), EMA(-), KI67(30%+), CD30(-), CD15(-), ALK-1(-), CD56(100%++), GranB(-), Per(-), TIA-1(scattered+), CD4(100%+++),CD8(-).
