AML with histiocytic differentiation is a morphologic variant of acute monocytic leukemia. The cells are large, pleomorphic, and deeply basophilic. There are prominent nucleoli. The cytoplasm contains vacuoles of many sizes. The over-all appearance is reminiscent of tissue histiocytes causing some to propose a separate FAB sub-classification, M5c.
