The patient is a 63 year-old male found to have lytic bone lesions. An IFE/SPEP was performed and showed an M-spike that was IgA lambda. A bone marrow biopsy was performed and showed numerous round globules in both the aspirate and core biopsy. The plasma cells were positive for CD138 . The aspirate smear shows occasional cytoplasmic round bodies within plasma cells, characteristic of Russell bodies.
