A 63-year-old male with a history of hepatitis B, hepatitis C, and follicular lymphoma grade IIIa, t(14;18) negative. He was receiving Rituxin for treatment of his lymphoma. He presented with shortness of air and a persistent cough. A diagnostic and therapeutic thoracentesis was performed. Small mature lymphocytes and red blood cells are dwarfed by large pleomorphic cells.
