The patient is 54 year old woman with circulating abnormal lymphocytes with a phenotype consistent with NK cells.

The patient has had lymphocytosis for years with only mild cytopenias.

The cells are medium in size with irregular nuclear contours and moderate cytoplasm. Many of the cells have distinct granules.
