Numerous large lymphoid cells in the peripheral blood is distinct nucleoli and scant to moderate amount of cytoplasm is characteristic of B-cell prolymphocytic leukemia (B-PLL). This entity can be seen either as a transformation of CLL or as a de novo disease. In the latter situation, blastoid transformation of mantle cell lymphoma is often a consideration and an underlying t(11;14) should be excluded before making the diagnosis of B-PLL.
