Atypical Pelgeroid nuclear changes in advanced HIV. In this setting, rather striking nuclear abnormalities may be observed in the neutrophils and may affect erythroid and megakaryocytic cell lines in the marrow without any underlying clonal myeloid process. This is related directly to the HIV and one must be cautious before rendering a diagnosis of MDS in the setting of HIV.
