Neutrophils with prominent hypolobation and Pseudo Pelger-Huet cytomorphology in a patient seen in the posttransplant setting. This patient was on mycophenolate which is sometimes known to induce such changes.

One must be cautious not make a diagnosis of myelodysplastic syndrome in the setting.
