A 55-year-old male with history of multiple myeloma presented to the hospital with lower right back pain 3 years after chemotherapy. He completed treatment including Velcade, Dexamethasone, and Revlimid before being lost to follow up. Imaging revealed a 6x5cm mass eroding the sacrum and ilium and extended into the epidural space of L2. Biopsy revealed images above: 

-H&E: compacted sheets of enlarged plasma cells with profound nuclei
- CD138: +
- Kappa-ISH: +
- Lambda-ISH: - 

The images are hallmark of plasmacytoma
