A 34-year-old diagnosed patient of systemic lupus attended emergency ward with rash, pain, and tenderness in joints. A complete haemogram revealed mild anemia with lymphocytosis. Peripheral smear observation found out the presence of reactive large granular lymphocytes (LGL). The image shows a variant of LGL with an aggregation of granules in the cytoplasm.
