A 27-year-old female presented with back pain, anemia and thrombocytopenia. Hb 80 g/L, WBC 8x10⁹/L, platelet 5x10⁹/L, creatinine 113 μmol/L. ADAMTS13 level <5 IU/ml. Blood film showed red cell fragmentation, polychromasia and thrombocytopenia. She was diagnosed with immune TTP. She was treated with urgent plasma exchange, methylprednisolone, rituximab and caplacizumab with complete recovery of full blood count and ADAMTS13 level.
