A 21-year-old male, with lumbosacral portions pained for 2 months, were admitted to a hospital. MRI study revealed abnormal signals in part cone of thoracic and lumbar vertebrae. Bone marrow smear showed monotonous infiltrate of large cells, these cells had loose chromatin nucleus, with abundant cytoplasm contained a variable number of vacuolations. The trephine biopsy was replaced by large cells. Immunohistochemical staining of the neoplastic cells were as follow: DES(+), VIM(++), CD56(++), LYSO(+), S100(+/-), CD45RO(-), SYN(-), CD1a(-), HMB45(-), CD99(-), MPO(-), CD68(-), CD61(-), LCA(-), CD20(-), CD79a(-), BCL-2(-), CD10(-), CD3(-), CD5(-), TdT(-), cyclinD1(-), CK(-). Rhabdomyosarcoma is the most common pediatric soft tissue tumor, there were two main histopathologic subtypes: embryonal rhabdomyosarcoma and alveolar rhabdomyosarcoma. Herein we presented a case of bone marrow with embryonal rhabdomyosarcoma metastasized.
