Bone marrow biopsy (40×) showing multifocal, dense infiltrates of numerous small intracellular and extracellular yeast forms consistent with Histoplasma capsulatum. The organisms appear as round to oval bodies with surrounding clear halos and are highlighted on periodic acid–Schiff (PAS) and Gomori methenamine silver (GMS) stains, confirming fungal etiology in this case of disseminated histoplasmosis.
