Peripheral blood smear demonstrates a markedly hypersegmented neutrophil with approximately ten distinct nuclear lobes connected by thin chromatin filaments. Surrounding erythrocytes are macrocytic with mild anisocytosis and occasional macro-ovalocytes. These morphologic features are characteristic of megaloblastic anemia resulting from impaired DNA synthesis, most commonly due to vitamin B12 or folate deficiency.
