H&E stained sections of the trephine core show evidence of marrow fibrosis, with ‘streaming’ of hematopoietic elements and dilated sinuses. There is trilineage hematopoiesis including increased megakaryocytes. Megakaryocytes include atypical forms with hyperchromatic nuclei.
