A 27-year-old Caucasian female with a history of Systemic Lupus Erythematosus presented with bilateral knee pain and swelling. Fluid analysis showed a nucleated cell count of 9,451 cells/uL with a differential of 61% PMNs, 17% lymphocytes, 18% macrophages, and 4% synovial lining cells. A Wright's stained smear of aspirated fluid from her right knee is shown. Two of the PMNs (arrows) contain amorphous globular inclusions consistent with LE cells.
