A 52-year-old African-American female with no significant comorbidities developed fever with chills a few days after returning from Jamaica. Her blood counts revealed a white blood count of 2.4 with granulocytes at 55%, hemoglobin of 12.9 gmd/dL, and a platelet count of 106. This unusual dividing cell was seen on peripheral smear. Evaluation for specific infectious etiology was unrevealing. She recovered uneventfully with empiric antibiotic therapy.
