Atypical cells of ATLL involving bone marrow aspirates. Often times, the cells look more pleomorphic with vacuolation and not morphologically very different from anaplastic lymphoma cells or plasmablasts on aspirates without obvious clover leaf cytomorphology.

This case was positive for CD3 and CD4 with complete loss of CD5 on the T-cells consistent with a neoplastic T-cell process.
