A patient with chest pain was referred to our center. Complete blood count (CBC) and peripheral blood smear examination revealed a leukocytosis (35000/μl) and a considerable number of hypersegmented neutrophil (>6% with more than 5 lobe). Platelet count of patient also was high (620000/μl). Precise examination of peripheral blood smear showed a number of giant platelets. Platelets were as large as a red blood cell or larger. Giant platelets can be observed in a number of disorders including myelodysplastic syndromes, essential thrombocythemia (ET), Bernard-Soulier syndrome, gray platelet syndrome and May-Hegglin anomaly. Giant platelet, randomly can be observed in normal individuals.
