P53 immunostain on the one core biopsy shows numerous abnormally positive cells outside of lymphoid aggregates within erythroid precursors and megakaryocytes. Notably the CLL component is also incidentally positive for p53. Normal bone marrow shows <1% p53+ cells and the observation of numerous p53 positive cells is consistent with a diagnosis of therapy-related myeloid neoplasm.
