Photomicrograph of a peripheral blood smear demonstrating multiple tear-drop shaped erythrocytes (“dacrocytes”). The associated bone marrow biopsy demonstrates numerous atypical, clustered megakaryocytes with cloud-like nuclei and cellular streaming suggestive of fibrosis. Upon further investigation, this patient was diagnosed with fibrotic stage of primary myelofibrosis.
