A 50 years female presented with bony pains, anemia and severe thrombocytopenia. Biochemical investigations revealed hypercalcemia. Bone marrow examination showed hypercellularity with many osteoclastic giant cells. Trephine biopsy showed distorted architecture with paratrabecular fibrosis. Marked osteoclastic activity, tunneling and prominent osteoblastic rimming was noted.

Parathormone level was markedly high at >2000 ng/L. A parathyroid adenoma was detected on imaging which was subsequently operated.
