Circulating atypical lymphoid cells with immunophenotype of chronic lymphocytic leukemia. The neoplasmic cells interestingly show cytoplasmic vacuolation. Studies have shown differential metabolic pathways in CLL cells compared with normal B-cells analogous to adipocytes and myocytes. There is also cytochemical and electron microscopic evidence that these vacuoles store lipids (Rozovski U, et al. Mol Cancer Res. 2015).
