1. A 3 year old male child complained of abdominal distension since 3 months and history of weight loss since 6 months.

2. On physical examination, he had pallor, frontal bossing and splenomegaly [8 cm below left subcostal margin].

3. His hemogram showed severe anaemia (Hb 2.9 g/dL), while peripheral smear showed moderate anisopoikilocytosis with marked reticulocytosis [4.8%]. Hemoglobin HPLC showed raised fetal Hb (HbF – 8.7%) and HbA2 (5.5 %) which is diagnostic of ß thalassemia intermedia .
