Mixed field Seen after O Rh Positive Red Cell Transfusion

1. During trauma setting a 24 year old male patient was given two units of O Rh positive red blood cells (RBCs). Later on patients’ blood typing he was found to be A Rh positive with “mixed field” (Mf).

2. On blood grouping patient typed “Mf” implicating that the transfused O Rh positive RBCs were not agglutinated by the anti-A monoclonal reagent that was used to type blood.

3. We could further see clumps of the patient’s A Rh positive cells along with free, un-agglutinated O Rh positive red cells in the background.
