An 89-year-old lady presented with general deterioration.  Hb 102g/L, WBC 59x10^9/L (neutrophil 6.04x 10^9/L, lymphocyte 0.89x 10^9/L), Platelet 19x 10^9/L. Blood film showed bilobed promyelocytes without Auer rods, in consistent with microgranular variant of acute promyelocytic leukemia. Flow cytometry identified 95% abnormal myeloid precursors. They are CD34-DR-CD117+CD33+cMPO+. Urgent FISH identified PML-RARA in 96% cells analysed. She was too unwell for treatment and was managed with palliation.
