Bone marrow biopsy- CD3 and TIA1 highlight the typical interstitial and intrasinusoidal infiltrates of neoplastic cytotoxic T cells in LGLL. These neoplastic cells express CD8 with frequent loss of CD5 and CD7. Granular staining is seen with TIA1, a cytotoxic marker. 

Sinusoidal infiltrates may be observed in other T-cell processes (such as hepatosplenic T-cell lymphoma, aggressive NK-cell leukemia, anaplastic large cell lymphoma) as well as B-cells lymphomas (splenic marginal zone lymphoma, intravascular large B-cell lymphoma). There is a fairly frequent association of T-LGL expansions with autoimmune diseases, particularly with Rheumatoid arthritis. Clonal proliferations are often associated with cytopenias and harbor underlying STAT3 or STAT5B mutations.
