A  47 year old Iraqi man presented with a history of two weeks fever and pallor.
▪️His CBC showed leukocytosis WBC count 34x 109/l, Hb 5g/dl, Platelet 60 x 109/l.
▪️Blood film revealed many leukemic promyelocytes ( hyper and hypo granular cytoplasm) with many Faggot cells  and few leukemic promyelocytes showed intracytoplasmic crystal like inclusion . The possible composition of these inclusions is unknown.

▪️Flowcytometry immunophenotypic analysis of peripheral blood sample revealed the presence of distinct cell cluster comprises( 89% ) of all gated events with high SSC signal and having a FSC with the majority of cells falling into the moderate size cell range.
The neoplastic cells expresses: cMPO, CD33, CD11b, CD13 and CD64 .
The neoplastic cells lack: CD34 , HLA-DR and CD117. So the morphology, IPT along with genetic study goes with the diagnosis of Acute promyelocytic leukemia.
