A middle-age lady known case of cold agglutinin disease (CAD) presented with hemolysis. The peripheral blood smear showed a very rare circulating megakaryocytes.

The presence of such finding is indicating an increased megakaryocytic differentiation. Thus, a thorough investigation is needed.
