A 14-year-old male patient was admitted to our service with fever and bleeding. The complete blood count showed hemoglobin 10.2 g/dL, WBC 658x10⁹/L and platelets of 35.0x10⁹/L. Peripheral blood smear showing 75% of blasts with small to intermediate size with condensed chromatin and no granules, scant cytoplasm, (Wright stain, original magnification, x1000). Flow cytometric analysis revealed positivity for CD2⁺⁺, CD3⁺, CD5⁺, CD7⁺⁺⁺, CD8⁺, CD13⁺, CD34⁺, CD45⁺⁺, CD99⁺⁺ and TCRalpha/beta⁺ antigens. Immunophenotyping was compatible with T-cell acute lymphoblastic leukemia.
