26 year old female presented to the Emergency department with complaints of breathlessness evaluated with basic blood and biochemistry found to have hemoglobin of 3.5 g/dl with markdly high MCV and MCHCand  nucleated rbcs were 300/100 wbcs.

Peripheral smear examination showed marked autoaggluntination with presence of Markedly increased nucleated rbcs , polychromatophils and microspherocytes .

reticulocyte count was done showed about 17% and Directs coombs test was postive 3+.

Diagnosis of autoimmune hemolytic anemia was given on perripheral smear examination.
