A 79-year-old female presented with WCC 390 x10^9/L. Bone marrow smear was effaced with large blasts with prominent nucleoli and occasional Auer rods. FISH identified t(8;21). Myeloid NGS showed NRAS mutation. She was treated with leukopheresis, hydroxycarbamide, followed by venetoclax and azacytidine to complete morphological remission.
