Peripheral blood smear showing hairy cells characterized by medium sized mononuclear cells having abundant pale grey cytoplasm with fuzzy borders. Nuclei have open chromatin with smooth borders and nucleoli are inconspicuous.  

Hairy cell leukemia is an indolent B cell neoplasm presenting with pancytopenia and splenomegaly. BRAF V600E mutation is seen in nearly all cases.
