Lack of HLA-DR expression in the setting of acute myeloid leukemia is often characteristic of acute promyelocytic leukemia (APL); however, this feature may be seen in other molecular subtypes of AML, characteristically in a subset of AML with NPM1 mutation. This case represents an example of AML with NPM1 mutation with blasts lacking HLA-DR expression.
