A 51-year-old woman with a recent history of RhD incompatible blood transfusion presented to the clinic with jaundice, severe anaemia (HB=3.6g/dL), thrombocytopenia (PLT=2x103/uL) and leucocytosis (WBC=20.80x103/uL) but normal PT (12.80secs) and PTT (33.8 secs). The liver function test showed elevated total bilirubin of 28.50umol/L. The peripheral blood film showed features of extravascular haemolysis characterised by marked polychromatic cells, spherocytes, nucleated red blood cells and RBCs aggregates/clumps. Extravascular haemolysis that occurs within the confines of the reticuloendothelial system is a typical hallmark of delayed haemolytic transfusion reaction. The direct antiglobulin test was positive as well.
