71-year-old male with history of acute myeloid leukemia presented with acute respiratory failure and hemoptysis. Bronchoalveolar lavage (BAL) revealed hemosiderin-laden macrophages (panel B; black arrow; original magnification x1000; Wright-Giemsa stain). In addition, macrophages with erythrophagocytosis are also seen (panel B; white arrow).
