A 66 year-old man presented significant weight loss, bone pain, fever and anemic syndrome. His blood count showed hyperleukocytosis with 340,000 per μL, hemoglobin of 6.6 g/dL and platelet count of 33,000 per μL.

Peripheral blood smear with Wrigth-Giemsa revealed 90% blasts with a cup-shaped nuclear morphology which is significantly associated with mutations in FLT3 and NPM1 genes.
