Figure 1 Upper Left shows Chest X ray of the patient with moderate pleural effusion. Upper right shows photomicrograph at 200X revealing 80% plasmacytoid cells in pleural fluid with reactive mesothelial cells. Lower Left   shows multinucleate atypical plasma cells. Lower right shows atypical plasma cells with reactive mesothelial cells.

Figure 2 Pleural Fluid immunophenotyping revealed (A) Plasma cells show positivity for CD38 and CD138. (B)The abnormal plasma cells do not express CD19. (C)The plasma cells show moderate CD56 expression .(D)The abnormal plasma cells show lambda restriction.
