93-year-old male presented with raised blood pressure and hip pain. Full blood count showed markedly elevated white cell count of 225 x 109/l and severe thrombocytopenia (51 x 109/l).  Peripheral blood film morphology shows circulating blast cells with high nuclear to cytoplasmic ratio, bilobed nucleus and occasional course basophilic granules.  Immunophenotyping demonstrated positive expression of CD33, CD7, CD117, CD34, CD13, HLADR, CD9, CD25, and negative expression for CD15 cCD79a, cMPO, cCD3 and cTDT.  This case represents the features associated with a rare case of Acute Basophilic Leukaemia.
