Polychromasia in a peripheral blood smear indicates an increased presence of immature red blood cells (reticulocytes), which appear larger and bluish-gray due to residual RNA. This finding suggests a heightened bone marrow response to anemia, commonly seen in hemolytic conditions. Our case involves a 56-year-old male patient diagnosed with warm autoimmune hemolytic anemia (WAHA). Polychromasia is a hallmark feature in WAHA, reflecting the body’s attempt to compensate for immune-mediated red blood cell destruction.
