Peripheral smear of a 67 year old male with relapsed and treatment refractory acute myeloid leukaemia, myelodysplasia related (WHO Classification 2022) showing dysplastic neutrophils with hypolobated nuclei and multiple, cytoplasmic Auer rods. Auer rods are typically associated with acute promyelocytic leukaemia (APL), including following the treatment with arsenic + ATRA, AML with maturation and mixed lineage leukaemias. The presence of Auer rod inclusions in neutrophils is a rare feature of granulocytic dysplasia, representing the atypical coalescence of primary neutrophil granules.
