A 15-year-old male presented with marked leukocytosis (WBC: 118,000 cells/cu.mm) with 85% blasts and thrombocytopenia. Peripheral smear (Leishman stain, low and high-power view) shows circulating blasts with high nuclear–cytoplasmic ratio, fine chromatin, inconspicous nucleoli and scant cytoplasm.  Background RBCs are predominantly microcytic and hypochromic with anisocytosis; platelets are reduced. Flow cytometry revealed blasts positive for CD10, CD19, cCD79a, and HLA-DR, with negativity for myeloid and T-cell markers, consistent with B-cell Acute Lymphoblastic Leukemia (B-ALL).
