Non-caseating granuloma noted on bone marrow trephine. Central epitheloid cells surrounded by cuff of small mature lymphocytes with a reactive background of plasma cells and eosinophils. Infectious causes of granuloma formation were excluded on immunohistochemistry. The patient had a history of Crohn's disease and primary sclerosing cholangitis and this was thought to be an extra-intestinal manifestation.
