The bone marrow aspirate shows numerous small, round yeast-like organisms with central clearing, seen both inside and outside histiocytes, with some demonstrating budding—features that are characteristic of Histoplasmosis caused by Histoplasma capsulatum; their intracellular location within macrophages and clustered distribution strongly indicate a disseminated (systemic) fungal infection, particularly involving the bone marrow.
