A bone marrow aspirate showing a Gaucher cell. These cells are characteristically large, round to oval, with a small, eccentrically placed nucleus. Their most striking feature is the abundant cytoplasm exhibiting a classic “crumpled tissue paper” or “wrinkled silk” appearance, caused by lysosomal accumulation of glucocerebroside due to deficiency of glucocerebrosidase. The cytoplasm is usually pale and may appear finely striated on microscopy.
