75 year old male presented to us with severe anemia ,Hemoglobin was 5 gm/dl .He had history of lower limbb gangrene for which he was planned for surgery latter developed severe anemia so refrrred to Clinical Hematology .On examination peripheral acrocynosis was present.PBF showed RBC agglutination not resolved at 37C.Direct coombs test was 4+.Monospecific showed positivity for IgG,C3d.Cold antibody were positive.He was treated with Inj Rituximab weekly .

R -After 2 doses of Rituximab 

His symptoms resolved .
