Hypersegmented neutrophils are a morphologic hallmark of disturbed nuclear maturation, classically seen in megaloblastic anemia, but they may also appear in other conditions like myelodysplasia, and drug‑ or cytokine‑stimulated states such as hydroxyurea or G‑CSF exposure. The abnormal segmentation arises from impaired DNA synthesis and delayed nuclear condensation during granulopoiesis, resulting in neutrophils with six or more nuclear lobes or more than 3–5% of neutrophils showing five or more lobes.
