Microscopic examination of bone marrow aspirate smears reveal clusters of histiocytes containing multiple intracellular fungal organisms (A & B). On H&E staining, the yeasts appear as small, uniform, oval inclusions within histiocyte cytoplasm (C). GMS stain distinctly highlights the organisms, confirming their fungal nature (D). The overall morphologic findings are consistent with Histoplasma capsulatum infection.
