A 5 year old boy presented with a one day history of pallor, jaundice and lethargy. Haemoglobin was 52gm/L, reticulocytes 5.2% (90x109/L), bilirubin 168umol/L and Heinz bodies 54%. Blood film showed marked numbers of blister cells, bite cells and spherocytes. G6PD level was 2.2U/gHb (9-20) confirming male hemizygosity of G6PD deficiency.
