An 8-year-old boy presented with gradually increasing painless right neck mass. Ultrasound revealed multiple bilateral cervical lymph nodes measuring up to 8 cm maximum dimension. He had no other significant medical history. Histological examination revealed thickened capsule with loss of nodal architecture secondary to sinus histiocytosis containing numerous plasma cells and histiocytes showing Emperipolesis. S100 immunostain highlights the histiocytes showing Emperipolesis. GMS stain was negative for fungal organisms.
